Refer a Pupil
Anyone can refer a pupil but the referral should have been discussed with the school's SENCO.
You can phone us during term time. We will need to know the child's name, date of birth, disability, name and address of the school, school borough, home borough and the reason for the referral. You can also now complete the referral form online.
Once we have this basic information we will send a more detailed form to the school and ask for a recent Individual Education Plan, a handwriting sample and Occupational Therapy and/or Speech and Language Therapy reports if available. Once the form is completed by the school and returned to CENMAC we approach the home borough for permission to go ahead with the assessment, because they fund this and the charge for the equipment loan and ongoing support.
When we have their permission we contact the school to arrange the assessment.
Some boroughs (Islington, Kensington, & Chelsea) prefer to have referrals directly to them in the first instance. Please telephone us for further details.
